2. Services Description
Healthy Defense Plan provides healthcare services including but not limited to primary care, skin treatments, hair transplants, weight management, dental care, fitness & wellness, healthcare insurance, and life insurance. All services are subject to availability and may vary by location.
3. Patient Responsibilities
Patients are responsible for providing accurate and complete medical history, following prescribed treatment plans, attending scheduled appointments, and informing us of any changes in their health condition. Failure to do so may affect the quality and outcomes of treatment.
4. Payment Terms
Payment is due at the time of service unless other arrangements have been made in advance. We accept major credit cards, insurance, and offer financing options. Prices are subject to change without notice. A valid ID and insurance card may be required at the time of service.
5. Cancellation & Refund Policy
Appointments may be cancelled or rescheduled up to 24 hours in advance without penalty. Late cancellations may incur a fee of $50. Refunds for services are evaluated on a case-by-case basis. Insurance-related refunds are subject to the terms of your specific policy.
6. Limitation of Liability
Healthy Defense Plan and its affiliates shall not be liable for any indirect, incidental, special, or consequential damages resulting from the use or inability to use our services. Our total liability shall not exceed the amount paid by you for the specific service in question.
7. Intellectual Property
All content, logos, and materials on the Healthy Defense Plan website are protected by intellectual property laws. Unauthorized reproduction, distribution, or modification of any content is strictly prohibited without prior written consent.
8. Governing Law
These terms shall be governed by and construed in accordance with the laws of the State of Nevada, USA. Any disputes arising from these terms shall be resolved in the courts of Washoe County, Nevada.
